NOTE: You will be asked to input a CIPP number. If you have it, please provide it; however, if you don't then don't worry - just say "None", "N/A". or a string of zeros. We can add this later once you register the player with USA Rugby. (part 2 of the registration process)

Phoenix Fall PreSeason is at Innovation Academy in Alpharetta GA.

Phoenix Rugby requires that the following documentation be provided by every player. Please note that these documents are required both USA Rugby as part of a comprehensive compliance and risk management process.

Documents for Middle School players (U14, grades 6-8) to be provided directly to Phoenix Rugby:

Required: Phoenix Rugby Waiver (For a Minor under 18)

Required: Verification of School Enrollment/Grade (e.g., a report card, letter from their school, or another document verifying the player's current grade and enrollment; players that are home schooled can provide a copy of the Online Declaration of Intent that was submitted to the Georgia Department of Education and the most recent report card). 

Required: Age Verification (a copy of a passport or birth certificate).

Required: Copy of Insurance or Medical ID card (Required for Travel)

For High School Players:

Required: Phoenix Rugby Waiver (Minor under 18 or Adult 18)

Required: High School Grade Eligibility (For this, Phoenix needs a Verification of School Enrollment; this can be obtained by submitting a request to High School Registrar office to complete; for home schooled players, please provide a copy of the Online Declaration of Intent that was submitted to the Georgia Department of Education and the most recent report card). 

Required: Age Verification (a copy of a valid Photo ID such as a driver's license; Phoenix can also accept a copy of a passport or birth certificate). 

Required: Copy of Insurance or Medical ID card (Required for Travel)
